Wellness & Employee Assistance Program

Fort Recovery Industries, Inc.’s wellness program focuses on improving your overall health. Good health is important to you, your family, and to us! Whether your goal is to have more energy, lose weight, manage stress, or improve your diet, the wellness program can help you. By participating in the wellness program, you’re taking key steps toward achieving and maintaining good health.

Who is eligible?

All FRI employees are encouraged to participate in our voluntary wellness biometric screening and Primary Care Physician visit.

Why participate?

By completing both the Biometric Screening and Primary Care Physician visit, you can earn a premium reduction on your health plan!

New Hires:

All new hires are eligible to participate in the Wellness program for discounted premiums. The biometric screening and the physical exam must be completed and turned in within 60 days from the first day of eligibility.

Current Employees:

Current employees must complete both biometric and physical screenings by August 14th in order to be eligible for the discounted premiums the following year.

Biometric Screening

A biometric screening is a health assessment that involves measuring various physical characteristics such as blood pressure, cholesterol levels, body weight, and body mass index to assess an individual’s overall health and risk factors for certain medical conditions. Employees covered by the Fort Recovery Industries, Inc. Health Plan will have the opportunity to participate in an onsite biometric screening at no charge (this event is held ONCE annually). You are also able to complete the screening at the nearest LabCorp or your physician (If you are choosing to go to LabCorp, you will need to bring along the attached ‘Lab Corp’ form).

NOTE: Fort Recovery Industries, Inc. does not receive your personal results.

Physical Exam

Knowing your numbers is half the battle. Discussing your results with your Primary Care Physician (PCP) or another qualified healthcare professional is equally important. The second portion of the wellness program involves scheduling an appointment with your Primary Care Physician (PCP) to discuss the results from your biometric screening and any concerns or questions you may have about your health.

What needs to be returned to get the discounted premium?

If you choose to participate in the wellness program, you will need to complete a ‘Screening Results Form’ attached below. Please see the form for all details (what to fill out and where to submit completed forms).

Employee Assistance Program (EAP)

Fort Recovery Industries understands work and personal challenges can impact the wellbeing of our valued employees. As assistance to you and your family members (dependents, household members and parents/parents-in-law), we offer a confidential Employee Assistance Program (EAP) to help overcome personal, lifestyle and work-related challenges. This NO- COST benefit offers access to a network of trained counselors and specialists to help with any legal, financial, and work-life balance assistance.
