Company Paid Life and Accidental Death & Dismemberment

Fort Recovery Industries, Inc. is proud to provide a Basic Life and Accidental Death & Dismemberment benefit at no cost to all full-time Associates through New York Life. The life insurance benefit will be paid to your designated beneficiary in the event of death while covered under the plan. The AD&D benefit will be paid in the event of a loss of life or limb by accident while covered under the plan.

FRI will only pay for employee coverage. Spouse and child coverage are available as additional voluntary life benefits paid for by the employee.

Voluntary Life Insurance

Full-time eligible employees are provided the opportunity to purchase additional voluntary life benefits through New York Life. Dependent on the amount of elected coverage, your election could be subject to medical questions and evidence of insurability.

Employees can purchase additional life insurance on themselves in increments of $10,000, their spouse in increments of $5,000, and their children in increments of $2,000. The guaranteed amount for employees is lesser than 5x annual salary or $150,000, for spouse is $25,000, and for children is $10,000. Evidence of Insurability (EOI) is not needed unless the elected benefit amount exceeds guaranteed issue amount.

All voluntary life insurance premiums are paid for by the employee. Please see your Benefit Guide to calculate your monthly premium.

Disability Insurance

Disability insurance is designed to cover a portion of your salary when you are unable to work due to an accident or illness. Fort Recovery Industries, Inc. offers both the Short-Term and Voluntary Long-Term Disability plans through New York Life. Short-Term disability is employer-paid, while Long-Term disability is employee-paid.

What qualifies you for Short-Term Disability?

A short-term disability is an instance that puts you out of work temporarily, such as an injury, illness or procedure.
